Utah Concealed Firearms Permit (CFP) Course - REPLACEMENT APPLICATION
 
For Delmarva Defense LLC customers who lost or misplaced their Utah CFP Applications/Certificates of Completion that fall within 365 days of the original training date. Replacements must be approved by Delmarva Defense LLC prior to issuance. If approved, student must contact Delmarva Defense LLC within 24 hours to schedule an in-person meeting to verify identity and re-issue the lost or misplaced application/certificate of completion.
 
Student must provide a valid driver's license and/or valid state issued photo identification and the receipt/invoice from the original course date.
 
If the original lost or misplaced application/certificate of completion is recovered, it must be destryoed immediately per Delmarva Defense LLC and the Utah Bureau of Criminal Identification.
